At Midtown Dental, our office guidelines ensure that your visits to our office go as smoothly as possible.
Medical Updates
Changes to your general medical health have an impact on your dental health. It is important that all patients keep us up to date regarding their overall health, and on the medications they are taking. Be sure to bring a list of any medications you are taking, including dosages (this includes over the counter medication and natural health products) to your first visit, and continue to update us as things change.
Dental History
If you have been having regular dental visits, you should contact your previous dentist to see if there are current x-rays that could be forwarded to our office. If you prefer, please call us and we can do this for you.

We will review three key areas during your first visit so that we have a clear picture of your oral health and your needs.
A Review of Your Dental Care History
We will review your dental care history, as well as any concerns you currently have about your oral health.
A Complete Oral Exam with X-rays
We will perform a complete oral exam, including X-rays to help us assess the overall health of your smile.
A Full Oral Health Plan
We will prepare recommendations for your total oral health, which will include full oral maintenance, as well as the potential of additional dentistry services that we agree upon.
Needless to say, every patient is an important part of our team. So, if there’s something specific that you would like to discuss about your oral dental care, please let us know. We are happy to listen and help.
Important Information About Insurance Benefits
Remember that you are an important part of your dental care, and that treatment and care decisions should be made in collaboration with your dental care provider.
A dental plan can be a great help in covering the cost of dental care. However, it is possible that your dental plan may not cover the full cost of the your required treatment. By maintaining good oral care habits in your daily life, you are making a long-term investment in your own well-being.
Midtown Dental is pleased to submit, process and complete any necessary forms or estimates at your request; however, dental insurance is a contract between you and your insurance carrier. Therefore, it is your responsibility to know what your insurance covers and to inform our office of any changes to your insurance i.e. carrier, policy, identification numbers or plan maximums. This will insure that your claims are processed correctly.
Our practice will assist patients with most dental plans and assists our clients in receiving reimbursement from their insurance programs. If you have concerns regarding your insurance benefits, please do not hesitate to call our office and speak to our treatment coordinator.
Changing Appointments
Your dental appointments are reserved specifically for you. Although we will contact you prior to a scheduled appointment with a courtesy reminder, it is ultimately your responsibility to make sure that you arrive for the appointment on time.
If you need to reschedule a reserved appointment, we require a minimum of 2 full business days’ notice, this permits another patient to receive dental care in your absence. If you miss an appointment without giving notice, a missed appointment fee will apply.

Patients are responsible for paying their dental procedure fees at its time of services. We accept the following methods of payment.
